Odd information in order

Hello all,

I haven't found the pattern yet, but a couple orders are showing up as:

1146 - Table 'prefix_table.TABLE_PAYPAL_IPN' doesn't exist

select * from TABLE_PAYPAL_IPN where paypal_ipn_id = '0'

[TEP STOP]

Obviously, the portion in bold is the actual prefix and table name, but I changed them for anonymity.

Any ideas on where to start troubleshooting?

Oh, the other things that are interesting are:

1) On the orders that go through successfully, the summary screen says: "Payment Method: PayPal (including Credit Cards and Debit Cards)," while on these failed orders it says "Method: PayPal."

2) On the "Packing Slip" screen for the failed orders, it has the quantities of the products ordered, but 1 x and then it is blank. At the very end, it has -:
