How to add additional customer details to account creation

I need to add a couple text boxes to the account creation page.
This will be a clean install and I could go straight 2.2 or latest MAX whichever accomplishes what I need.

Below are what I need.

new items in bold.

Occupation (radio buttons?)
PT OT ATC MD PTA OTA
First Name
Last Name
Address 1
Address 2
City
State
Zip
Telephone
Company
Student ID
Phone
Fax
E-Mail
Password
Confirm Password

Any idea how to do this? Is there already a contrib that I am just not finding?

Yeah, I started there and went all through it. I tell you, I would have loved that 4 or 5 years ago when I got started. It was like going back in time, especially the common problems section. I have been using OSC since it was The Exchange Project.

There isn't anything about adding the features I am looking for unless I missed something.

I am just really surprised that in all this time there hasn't been more of a demand for additions to the customer details in the signup form.

Perhaps I am not asking the right questions, I cant think of any other keywords to search for.

Any other ideas?

I thought it was in there. I do remember reading how make those alterations years ago in the Osc Forums - from Linda I think. If I dig something up I'll post back here.
