feedback form?? customizeable?

ive been searching on forum and addons of oscommerce but i couldnt find the feedback form addon.. which could be customized to choice...
the thing im looking for is like this:

if someone can help me make the feedback form same as above.. please help me here..

Did you mean the CONTACT US feature that is included?

we do have a contact us page like u showed.. but we need another one.. as a feedback form.. similar to this:

do u knw anything like this?? or if u can help me make it?
im not good at php but i do know how to follow instructions properly

So....you want to add additional information like a Category/Subject and a URL?

Look at:

Super Contact us enhancement

Possibly look at: customer suggestions or Contact Us Email Subject
