Default adding required feilds to account

Hello Folks,
Has any one added additional required feilds to the create an account page. ie company required, tax id #, cell phone...
I have seen some older documentation in contribs to edit the modules/account_details.php to re arange some info
But, I do not have this file in ms2 max 1.5.
The closest match is affiliate_account _details ?
Are there any instructions on what pages to edit to add these options?
I know I will need to add columns to the db tables.

There is a contribution that ADDs a Tax Exempt... How ever with BTS - many files are split between the main file (*.php) and the template files (*.tmp.php) in the template directory. You have to look in both to find the proper code.
