Customer Info not in order (name, address, etc)

I can't figure out how to reproduce it, but about 1 of every 10 orders have no customer info at all. Name, Address, Phone, Email, all blank.

The product ordered, and price are correct, but no info.

Any ideas?

What payment method is used by those customers? Is there a difference between those orders and the others that appear correctly?

I do all my testing with "Cash on Delivery"

So that is the payment method used every time.

When I go live I will be using Authorize.net, I've tested it many times in "Test Mode", I just find it a pain to enter all the info every time I want to checkout.
