Confirmation e-mail

I do not get extra e-mail of the order when they order, I even set up a extra e-mail to be send and also not working, I change to smtp to send the e-mails and also not working, have somebody come to this issue, or is just something very wrong in my configuration? please any help apreciate.

I notice also that if it is a old custumer it will not send a e-mail, just the new custumers?
