Trouble making an account

I am trying to make an account and it keeps bringing me back to the account infomation page, and wont allow me to make a new account, anyone else having this problem and any idea on how to fix??

Is this a regular store account or an affiliate account?

Post the file that is causing the problem (for example: create_account.php), and the fields that seem to be causing the problem.
