Add-to-cart not adding selected product to shopping cart, it adds to the "quantity" of item already in Shoppingcart

Hello guys, I"m new here and I would appreciate any help.

I've been having this problem with a customer. He adds to his shopping cart and and after a couple items it starts with this problem. Instead of adding the new item to the cart when selected, it just adds 1 to the quantity of the previous item in his cart. I saw it happen once but but I can't get it to do it for me again even if I log in their account with the Master password. i just had him try it again and he is still having the problem. I'm using OsCmax2.0.
