MOD Country-State Selector

I have a store I migrated from osc2.1 to oscmax2. I have completed the db migration and have fixed about 99% of the problems.

One I'm still having and can't fix is on the page checkout_shipping_address.php. For some reason, when the page loads, under the "New Shipping Address" section, the State field has about 50 dropdown boxes displayed. Each dd has one more state appended to the option list. Now, if I choose another country from the Country dd, the State dd loads ok. And if I then choose "United States" from the Country dd, the State loads fine.

Also, the page <title> says "NAVBAR_TITLE". A clue to what's wrong? There's also a php close tag "?>" on the screen right before the "First Name:" field.

Any ideas how to fix?

Interestingly, the State dd loads fine on address_book_process.php, an almost identical screen to the "New Shipping Address" section on checkout_shipping_address.php.

I saw this bug fix...

It fixes the dangling "?>" tag and the <title> tag. Thanks. But it does not fix the repeating State dropdown field problem. Any ideas on that one?
