adding cvv code to credit card module

i tried to add the cvv and now when you try to check out i get a error

Try this contribution if you want a simple way to add the CVV field to osc's generic cc.php.

when i finally hit comfirm i get this error:

the fatal error happens when the customer confirms there order, but everything with the site and order is working ok. the order goes through and all the order data shows up in the admin area. can someone please tell me what this error is?
