Help with Monthly Report

I am looking for a contribution or a way if someone has already done it to be able to print out a report that just gives Customer Name, Order Number Date and Sub total or total. similar to clicking on orders in admin.

This way I can print the orders by customer every month for our files in an easy form. Also by year if needed for books.

I have installed a few contributions I found on my test shop but none have done the above so far. In my search I did like orders tracking 2.9b in the oscommerce contributions. It gives some different insights and only gave one simple fix error on install.

Anyway hope someone can help.
