I would like to know if there is a contribution or anyway to add a feedback request form to the shopping cart or any suggestion on where to place this form would be great.

Reason:
You have a potential customer adding products to the shopping cart, then the customer is gone.

I would like to place a feedback request form to let me know why the cumstomer did not complete his/her order.

I tried adding the contribution: If you dont buy tell us why but I do not get any emails, if someone has a fix for that, that would be great to.

Any help would be appreciated.
