Default creating vendor application form and errors!

Call to a member function on a non-object in /home/hsphere/local/home/arkmusic/arkmusic.com.au/catalog/vendor_form.php on line 95
Whenever I try to run the form module I created I get the above error.

Attached is the form - can anyone advise?
