catalog/checkout_shipping_address.php isn't loading

I'm having trouble on both of my osCommerce sites:

When the customer goes into the checkout process and then clicks the "Change Address" button under the Shipping Address (checkout_shipping.php), the checkout_shipping_address.php page loads as blank. I get the background image, but nothing else.

Any ideas about this? Has anyone else tried this?
