Team registration? How would you do it?

I am working on a site for an indoor soccer arena. They will be selling soccer equip and apparel among other things. They would like to be able to take payment for league fees and registrations through the website.

This is what I would like... any ideas if I am barking up the wrong tree? Or a simpler way to do it?

You will have to bear with me becuase in the half dozen OSC shops I have put together I have never once dealt with the attributes. So, I am unfamiliar with them and was hoping someone could tell me if the below is possible and save me from making a mess of attributes?

Yea Atributes...You might want to also add Atributes with text boxes

This contribution specifically adds the ability to display options with text input boxes. This allows a store owner to collect customization/personalization information for a product from the end user. The contribution is written in such a way that the addition of other option types such as Radio Buttons should be reasonably simple to add.
