local install on my windows box - will try doing it onto my server and see how it goes - will give you ip address if it happens again so you can see

This can be fixed by going to catalog\includes\modules\authorizenet_direct.php and go to the end of the file put your cursor next to > and hit delete and then save. I can't help with the other....
