Customer Feedback Form

After installing the add_feedback box by 'Cooperman' with the instructions laid out by 'Blademaster' found the box didn't work or at least not on our web site. I have added a new update to the english.php, also includes Coopermans revised add_feedback. Very easy update, just replace or add as necessary.
