Payment Module for setcom.com

Package includes files & installation instructions for v2.1 and upgrade instructions for v2.0 to v2.1

Upgrade Changes:

1. Added code to disable module on checkout payment screen if OSC currency chosen is not supported by Setcom.
2. Moved code that adds the osc order num to the setcom table, from the checkout success page to the after_process function.
