Separate Pricing Per Customer

Full package that includes a bugfix in login.php (customer specific taxes exempt would still not work if you logged in as the test user).
Added the SPPC modified tax class for contribution "Optimize tep_get_tax_rate() method" (http://addons.oscommerce.com/info/2417).
