Based on feedback, we have changed some of the language, fixed numerous bugs and added a process quote button to avoid confusion for mouse reliant customers.

1. Added "Process quote" button so that customers may click a button or hit enter to process the form. Some customers are confused or don't know to hit enter otherwise.
2. Updates to language file for better button descriptions and a description change above the "change" button

1. Changed form to require only zip code as USPS, UPS & FedEx only require this information to deliver shipping quotes
2. Formatting changes to show logo at top, shipping information on upper left, address change ability (form reset), product listing, all available shipping options. Show shippers logos and table format for options. Continue button (form reset).
3. Added instructions and made install package zip
4. Error/bug fixes
